·       Athletic Registration – ONLINE on Family ID follow the link below

o   CURRENT Physical, Medical History and CHSAA Form – (online at Family ID, link above)

§  Physicals are good for 365 days from date of physical. Denver East MUST have a current physical on file in the Athletic Training Office to participate.

o   Parent Permission & Emergency Form – (online at Family ID, link above)

o   Player Participation Agreement – reviews rules, expectation and consequences regarding, eligibility, attendance, behavior and drugs/alcohol. – (online at Family ID, link above)

o   Pay to Play – $60 or $10 if on Free and Reduced Lunch

·       MUST BE Eligible – Transfer Eligibility and Academic Eligibility REQUIRED to participate.

o   If you attended any other high school other than East High School in the last 12 months you need to see Lisa Porter.

·       ATHLETIC CLEARANCE CARD will be issued when all things are complete. This is REQUIRED to give to the coach so the coach knows you are cleared to participate in tryouts.

o   Fall Athletic Registration- August 7th-11th from 11:45am-12:30pm (after Freshman Academy) on the 2nd Floor by the Treasurer’s Office. 

August 14th – 17th – 2:30-3:30pm (but we will stay until all students are taken care of) on the 2nd Floor by the Treasurer’s Office


Athletic sports that are sanctioned by the Denver Public School Board of Education and meet the criteria may exempt participation however, such exemption WILL NOT reduce the number of hours required to earn a diploma.

·       Participation in ONE FULL Sports Season = 5 semester hours of PE Requirement

·       Student MUST maintain academically eligibility for the ENTIRE Season

·       Students MUST request the waiver during the same semester that the sport is played.  Please see your counselor for the form. The form must be signed by the Athletic Director and Counselor.

Note:  Club Sports and non-East/DPS sports DO NOT fulfill the PE Waiver requirement.



NOTE: Players might be asked to come to a different tryout time than what is listed for their grade.

In order for a student to participate in sports there are three (3) required forms + Fee of $60.

1.     Physical Form

2.     Emergency Contact Form

3.     Player Agreement

4.     Athletic Fee of $60 payable to East High School.

{Student athletes are not permitted on the courts until ALL paperwork has been submitted.}
